FamilyWise’s fully-credentialed High-Fidelity Wraparound youth case management program lifts up family voice and choice so that we can “wrap” the right support around youth and their families. We help you tap into a network of informal “natural supports,” from neighbors to friends to family members who can all contribute to a healthy home environment.
What is Wraparound?
High-Fidelity Wraparound (commonly called Wrap) is a nationally-recognized model aimed at helping youth with significant behavioral, emotional, and mental health issues. It “wraps” a family in support and resources. This is done by:
- Allowing youth to remain in their communities rather than being placed in a correctional or residential treatment facility.
- Establishing a team of formal and informal supports.
- Developing a plan to meet their goals and to build long-term support from the community that continues long after Wrap ends.
- Working on that plan step-by-step with successes that build the confidence of the family to achieve their goals.
- Supporting the whole family in receiving the resources that they need.
The wraparound process ends when the whole family feels more stable and believes they can continue to work towards their goals on their own.
How Does Wraparound Work?
After you have been referred, our staff will work with you to figure out the best group of people to be on your Wrap team. Together, we will put together a Wraparound Action Plan that meets your needs in a range of areas, from education to mental health to safety and more.
