In FY2024, Carlton County Children’s Mental Heath and Family Services Collaborative received a Community Resilience Initiative micro grant to develop an online hub of community resources and interactive maps to foster connection and wellbeing. Through comprehensive community input, a theme had emerged–Carlton County residents wanted enhanced connections to community, essential services, and organizations. Some were feeling disconnected from community, while others were struggling with basic needs and uncertain of how to seek assistance.

In response, the Well Within Reach team embarked on the creation of a project to update and enhance existing databases of resources, create interactive mapping of resources, and develop a community connections calendar, all to live on a new website, the Well Within Reach Hub. This project was funded by multiple sources, with the micro grant covering a portion of staff time for an existing community navigator.
