Please send the following to FamilyWise after EVERY presentation you facilitate: 

• Cover Sheet
• Sign-In Sheet: You can submit your own sign-in sheet or use the FamilyWise sign-in sheet
• Participant Surveys: You can submit paper surveys or let us know that you sent participants an electronic link

Certification Paperwork

Please send the following to FamilyWise after THE FIRST TWO presentations you facilitate:

• Self-Reflection Form 
• Peer Feedback Form: Ask your co-presenter to send this to FamilyWise on your behalf

Presenter Demonstrations

After you have completed two presentations and submitted all of your presentation paperwork, you are eligible for the last step in the certification process — completing a Presenter Demonstration Session.
